Inpatient Amputation Rehabilitation Program
The Amputation Rehabilitation Program at New England Sinai Hospital provides a patient/family-focused program of education and treatment that emphasizes each patient's unique strengths, abilities and preferences. The Program's goal is to maximize the patient's level of functional independence to be consistent with the demands of his/her community environment. The program places a strong emphasis on patient and family/support system education and advocacy to optimize successful outcomes. Our patients, on average, are 72 years old, and are usually discharged to home, or to a skilled nursing facility when they need additional care. In 2007, 67% of the Program's Amputee patients were discharged to home, and 67% achieved their predicted outcomes.
Our Amputation Rehabilitation Program:
·Treats conditions and complications related to limb loss,
·Reduces risk factors for further amputation,
·Facilitates functional independence and performance,
·Identifies and meets needs for prosthetic care,
·Facilitates psychological and social coping and adaptation skills,
·Facilitates community integration and participation in life roles,
·Provides services for families/support systems.
Program Admission Criteria:
·Primary Diagnosis of Amputation
·Adult population
·Rehabilitative and/or medical acuity to require hospital level of care
·Patient's medical status must be stable enough to be manageable by the facility
·Presence of physical/cognitive impairments resulting in activity limitations & participation restrictions
·Ability to achieve goals within a reasonable period of time
·Ability to participate in an intensive interdisciplinary hospital-level rehabilitation program
·Patient's psychological and behavioral status and cultural needs must be manageable by the facility
·Identified viable discharge plan
·Appropriate funding source
