Adult Day Health Program
Eligibility
Each applicant is evaluated by the program staff on an individual basis.
To be eligible for the program, participants must:
- Be 22 years of age or older
- Need day health care services a minimum of two days a week
- Have a personal physician or primary health care provider in the community
- Have a place to return to at night, and a family member or friend to provide appropriate care.
- Have no major behavioral problems
Tuition - Scholarship Information
For most participants, program tuition is the responsibility of the participant or his/her family. Medicare does not cover adult day services. Medicaid, as well as the Veteran's Administration does cover adult day health services, and funding may be available through local Aging Services Access Planning services (ASAP), and some long term care insurance coverage. Rotary Community Corp Scholarship
Program participants of the Adult Day Health Program who can not afford the per day program and who do not receive financial assistance from the Federal or State Governments can be eligible for consideration for Rotary Community Corp Scholarships (RCCADHP). The RCCADHP is an approved Rotary International entity and is a cooperative effort of 17 Rotary Clubs and New England Sinai. Together the RCCADHP and Sinai Adult Day Health raise scholarship funds and coordinate the scholarship assistance program. For scholarship application, click here (PDF file).
