A Physician's Guide to Patient Referral to the Sinai Pulmonary Rehabilitation Program

Inpatient Program

Patients may be admitted from acute hospitals. In appropriate circumstances, patients are admitted directly from home. Call the admissions department at (781) 297-1600.

Outpatient Program

• If your patient is an inpatient at New England Sinai, include "Outpatient Pulmonary Rehab" as one of your discharge orders.
• From your office call (781) 297-1283
  Please fax as much of the following information as possible to (781) 341-9742.
• Diagnosis
• History of present illness
• Synopsis of the past medical history
• Latest complete PFT
• CXR
• Lab results (including ABGs)
• Current medications

You might consider Pulmonary Rehabilitation for your patient if any of the following are present:

• Recent exacerbation and hospitalization
• Increased hospitalizations, ED visits, office visits/calls
• Oxygen desaturation or diminished PFTs
• Use of supplemental oxygen
• Decrease in physical activity
• Complaints of dyspnea
• Disease is affecting the quality of life
• Psychosocial changes
